Pre-Clerkship Absence Form

This form must be filled out to request or report an absence from:

A mandatory activity
An examination
To request any other exception to the School of Medicine policy.

Class absences can only be approved by Academic Affairs.

In emergent situations, (i.e. less than 24 hours notice), you need to call the persons below and complete this form as soon as possible. Missed work and/or makeup work will be addressed/scheduled during normal business hours (Monday-Friday 8 a.m. - 5 p.m., excluding Health Science Center holidays) and may need to be handled once the student returns to school.

You must provide all requested information. All information is subject to an independent review and confirmation process.
